With the correct OTP you will be redirected to the actual form. Fill up the form with relavant and exact information. After successful submission an enrollment number will generate, keep that for further correspondence.

  • Photograph must be a recent passport style colour picture
  • Dimensions 413 x 531 pixels (within)
  • Size of file should be between 10kb-100kb

  Signature Image:

  • Dimensions 1024 x 768 pixels (within)
  • Size of file should be between 10kb-30kb
